Cough Worsens at Night? Here Are the Causes and Easy Ways to Control It

Many people experience worsening coughs at night, as if the body chose the time of rest to start a new battle with dry or phlegm-filled coughs. The respiratory system responds to the body’s biological clock and sleeping position, increasing airway irritation during nighttime.

Why cough worsens at night:
At night, cortisol levels drop, reducing anti-inflammatory protection, while white blood cells activate to fight infections, increasing inflammatory substances that cause nasal and throat congestion and frequent coughing. Body temperature also rises slightly, heightening feverish feelings or fatigue, especially during colds or viral infections.

Effect of sleeping position and gravity:
Lying on the back hinders mucus drainage from the nose and sinuses, causing it to accumulate in the throat and trigger cough receptors. People with gastroesophageal reflux may also experience stronger nighttime coughing due to acid reflux when lying down.

Environmental factors:
Dust, feather pillows, pet hair, or mold in the bedroom can increase respiratory sensitivity and trigger nighttime coughing. It is recommended to wash bedding weekly, use allergy-proof pillows, and ventilate the room daily.

Ways to control nighttime coughing:

  • Use warm steam before bed to moisten airways and reduce irritation.

  • Take a spoonful of natural honey 30 minutes before sleep to soothe coughs and improve sleep quality.

  • Use saline nasal rinse to clear mucus and relieve congestion.

  • Elevate head and shoulders during sleep to prevent mucus buildup and ease breathing.

  • For asthma or chronic obstructive pulmonary disease (COPD) patients, consult a doctor to adjust nighttime treatment with inhalers or anti-inflammatory medications.

When to see a doctor:
If coughing persists for more than two weeks, or is accompanied by wheezing, chest pain, or shortness of breath, it may indicate a deeper issue such as chronic sinusitis, reflux, or asthma. Coughs that frequently wake the patient should not be ignored as they may indicate a respiratory disorder requiring precise diagnosis.
